Q: How can I make a report of my survey database in the format:

A: With ClarisWorks, there are 2 approaches you can choose from. One is to create the fields Question Number, Answer and Person. Each record in the database would be the answer to a single question with the question number noted. A sub-summary layout part by Question Number with the Question Number field in the sub-summary and the Answer and Person field in the Body part would give you the printout you want. The downside to this approach is that all a person's answers aren't kept together in one record.
The other approach is to create a field for each question and also a layout for each question. One by one, you would switch to each layout, select all records and copy. By pasting into a WP document, you could assemble the desired report.
FileMaker Pro 2.0 offers a third approach. It can produce this report using repeating fields and a 2-database design where you import repeating fields from the data entry file and split them into separate records. This is demonstrated in the Survey Analysis file of the cross referenced article.
