If the issue does not go away when you restart your machine with Extensions off, it is likely that you have a corrupt font or a font that does not support Smart Quotes. Try switching to another standard Macintosh font like Times or Helvetica, to see if your Smart Quotes print properly.
If switching to another font allows the Smart Quotes to print, then you know that the original font is either damaged or does not support Smart Quotes. To check for damage, remove and reinstall the font and see if that fixes the issue. To see whether or not that font supports Smart Quotes, contact the font manufacturer.
