iTunes 2: Window Titles Are Blank

If your monitor is set to display 256 colors, window titles in iTunes may be blank.
Symptom

Window titles in iTunes are blank.

Solution

Increase the number of colors in the Monitors control panel (Mac OS 9), or the Displays pane of System Preferences (Mac OS X) to Thousands or more.
